c. Certification issued by the Regional Trial Court in the judicial district where the area applied for is located to the effect that the same is not involved in any judicial case (1 original copy)

d. Duly accomplished FLA or ASC application duly acknowledged before a Notary Public (15 original copies)

e. One (1) copy of the survey plan of the area under his/her name duly approved by the Director of Lands or Regional Director of Lands (Blueprint)
